In accordance with the ever-changing COVID-19 climate, Chase beauty is implementing numerous changes to our already extensive sanitizing and cleaning procedures. There are also adaptations to scheduling, length of time between clients, Personal Protective Equipment (PPE) and amenities within the building. 

 

Please review the NEW Chase Beauty Guidelines that are effective immediately. 

  • CONSENT FOR TREATMENT FORM must be filled out before you arrive to your appointment.

  • Please do not arrive early to your appointment.

  • When you arrive outside salon republic, please text me 661-993-2858 to let me know that you have arrived. I will meet you at the front and escort you back to my studio.

  • Your temperature will be taken before walking back to the studio with a no-contact infrared forehead thermometer.

  • Please make sure you wear a mask to your appointment. the mask will be removed at the start of your service, but must be put back on immediately after.

  • Please do not bring guests/children with you to your appointment.

  • Please limit talking if possible (other than necessary conversation as it pertains to your appointment).

  • Please maintain and observe social distancing when in the hallways and common areas of salon republic.

  • Please wash your hands/use hand sanitizer upon entering and exiting the building.

  • Service providers and guests will not be permitted on the premises if they exhibit any symptoms of being ill or under the weather such as: Fever, sore throat, coughing, sneezing, runny nose, vomiting. If you are worried that you might be sick, please reschedule your appointment. 

  • If you have been exposed to anyone that is sick, please reschedule your appointment.

  • All equipment, tools and surroundings will be thoroughly sanitized between each guest.

I understand that, because esthetics involves maintained touch and close physical proximity over an extended period of time, there may be an elevated risk of disease transmission, including COVID-19. Upon entering CHase beauty, I acknowledge that I am aware of the risks involved from receiving treatment at this time, I voluntarily agree to assume those risks, and I release and hold harmless amanda smith and chase beauty from any claims related thereto. I give my consent to receive treatment from amanda smith.
